New Client - Join Our Waitlist

We’re currently on a short waitlist for new clients, and we’d love to get to know your family! To begin the process, please complete our Contact Form. Once we receive your information, we’ll create a client chart and share the intake paperwork electronically, making it easy for you to complete from home. We may also need to verify insurance benefits and, depending on your insurance requirements, have a PCP referral on file. If a referral is needed, we’ll guide you through the process. Please note that your child’s intake paperwork and any necessary insurance verification must be complete before we can schedule an appointment. This helps us be ready to welcome your child as soon as a spot becomes available!
